    Is this an e-cargo bike has an extended frame and is designed for the primary purpose of transporting passengers or other cargo. This e-cargo bike meets the requirements listed on the City of Ashland's e-bike program website.

    I attest that the following requirements are met: The e-bike is rated Class 1, Class 2, or Class 3. The e-bike battery carries a warranty of at least 1 year. Bike was purchased on or after April 15, 2022. The rebate will not exceed 50% of the purchase price. E-bike meets all requirements currently listed on the program website. E-bike is new. E-bike is not a full suspension mountain bike. E-bike is not built from a conversion kit. E-bike invoice is paid in full and applicant has possession of the bike. The incentive application is submitted within 180 days of purchase. Rebate recipient must retain ownership of the e-bike for a minimum of 12 months from the date of purchase. The applicant resides at the Ashland Electric service address listed on this application. The applicant has consent to apply from primary utility customer, or an authorized individual under the account. The applicant is the purchaser of the bicycle. Only one e-bike rebate may be issued per Ashland Electric Utility account. Submitted program application and accompanying documents become the property of the City. Submissions that do not meet the program eligibility criteria will not be considered. Incomplete documentation may result in incentive ineligibility. The City is not responsible for documents lost or destroyed in transit or misdirected applications. The City reserves the right to request additional information or documentation to process an application. Incentives are issued on a first-come, first-served basis until funds are depleted. Check may only be issued to the applicant who purchased the e-bike. Terms, conditions, and availability of this program may change at any time.
